Most people write out combination dialing instructions in ways that only they can understand, and not for security reasons. It's because they don't know how to write them correctly.

I've been handed owner-written combinations that looked as if they were written by Tolstoy when all that was needed was something like this:
4 times left to xx
3 times right to xx
2 times left to xx
1 time right to stop

If your father was able to open that safe using the numbers you have, obviously you're not dialing correctly or the lock is not working right.

Instead of guessing and trying and failing repeatedly why not hire someone to come and service the lock and dial assembly AND show you how to dial correctly? I'm willing to bet your safe probably hasn't been serviced in the three generations of ownership you cited.

You could also scan backwards on this forum and read through some of the many previous posts about the same topic and maybe get some helpful clues.
