Yes, there are vault doors on Wall Street that are OVER 4' thick, closer to 5'-6' thick if memory serves me right. I used to service them when I worked for a bank servicing company. The amazing thing about these doors, is that 1 man can open and close them. The floor drops away to allow the door to swing open.

These doors are still in use to this day. A weight of 100 tons is entirely possible with these. I didn't try picking one up, but am quite sure that I couldn't. One location that I remember working on had at least 6 different vaults in at least 3 different sub-basements.

As for what they can stand up to, I would never be able to figure that one out. The only thing I know of is if it was shot by a 12" gun, then I don't want to be anywhere near it when it happens. The second thing I am pretty certain of, is if it gets hit, it's probably not going to open again. Consider the force of the impact, and the expansion of the door into the frame.
